I heard that the random were your pick/axe head flys off is now discontinued. Is this true? I haven't seen this random for a very long time now so I suspect this is true.

If that is true, wouldn't that mean there is new disconinuted items?

The axe head no longer falls off, due to the Dragon axe, but I believe pickaxe heads still do, but I am not certain on this point.

Axe heads don't fall off and all axe heads and axe sticks were deleted i believe.

pickaxe heads do still fall off though. it happened to me once recently

Yeah Pickaxe and Spade heads still fly off, but Woddcutting axe's don't
